Whether your loved one is in need of short term relief or a more permanent care arrangement, we take the time to learn about their needs and interests to tailor our services to meet them. With an RN on staff and carefully screened caregivers, our team provides many areas of expertise including Personal Care, Companion Care, Homemaking, Respite and Sitter Services for independent living. As part of our quality care services, we assign field supervisors who perform unscheduled supervision visits to help ensure your loved one is receiving the best care possible. This also gives you the opportunity to communicate any concerns you may find uncomfortable discussing with your caregiver. We take careful notes of each visit and alert you regarding any changes or concerns regarding your loved one. Our services are affordable, available 24 hours a day, 365 days a year, and our staff continually refines your loved ones personal care plan to ensure that they are enjoying the very best quality of life possible.

Apart from providing the client with home care, we also offer respite for family members or family caregivers. Home Care Evaluation for FREE
We do not charge you for the Home Care Evaluation that is conducted by a registered nurse or a care coordinator. Such is necessary to ensure that you receive the level of care that is required to preserve your very best health.

Periodic Care Plan Adjustments
From time to time, we will conduct an evaluation of your health and your response to the existing care plan. If needed, we will adjust your care plan to accommodate any changes or improvements in your health.

Getting started with Smart Choice Home Healthcare is a simple and convenient process.
When you’re ready, give us a Call at 216-230-9245 and a staff coordinator will meet with you and your loved one for your FREE in depth evaluation of their needs and lifestyle. You will be advised on the available home care options, payment arrangements, and other particulars related to their care plan. Your loved ones needs and wishes will be carefully matched with a caregiver and schedule to help improve their quality of life. Prior to the start of services, you will be given a chance to meet the caregivers or a set of caregiver candidates to choose from. In an event that your care plan needs to be adjusted, we are there for any changes you need. We also provide monthly assessments with our Registered Nurse Care plan.
We also offer caregiver training for family, and friends. Our Nurse aide and Home health aide training is available for those wishing to care for your love one or those who wish to join the healthcare field.
